Dr. Moore, we met in AMS Miami Cadaver lab last month. I am Dr. Maiti, from Mexico. You showed me the Miniarc technique. Well, returning back I put my first Miniarc. I felt it well with the technique. I had no problem putting in the sling. The patient had no pain. I saw her 10 days after, good healing, no pain, no voiding problem and no incontinence. I had the pleasure of evaluating [a patient] on 12/9/08 for complaints of urinary incontinence, prolapse, and sexual dysfunction. She was previously evaluated at… Massachusetts General Hospital and Dr. Miklos at Atlanta Urogynecology Associates. On my evaluation, she was noted to have prolapse and was associated sexual dysfunction. Simple CMG was deferred but she has demonstrated stress incontinence on previous evaluation. I had a lengthy discussion with her regarding treatment options including surgical correction.

Based on my evaluation of the patient and my knowledge of the other providers she has seen, I would think she would best be served by continuing treatment with Dr. Miklos. I am familiar with his work and have had the opportunity to refer several patients to him with similar complaints. Not only is he an accomplished pelvic surgeon, he specializes in sexual dysfunction related to anatomic changes of the vagina. Given her symptoms of sexual dysfunction and difficulty achieving orgasm, she would be be served by his clinical experience and expertise.

It's true that one must first be skilled at an abdominal procedure, in terms of anatomy and technique, prior to attempting it laparoscopically.

By the way, the idea of lateral posterior vaginal wall defects is revolutionary and anatomically sound.
